We do not ask for NHS numbers, GP clinical records, or a symptom list that could be read as a diagnosis.
How it is stored
Launch stack:
- You submit via Fluent Forms on this WordPress site.
- Fluent Forms stores the entry on the site’s hosting.
- A copy is emailed to the site operator, PASECOM GROUP LTD.
We do not use Meta (Facebook/Instagram) or TikTok pixels on medical or pathway forms, including the quiz and get-help form.
When we share data with a clinic
We share your get-help details with a matching partner clinic only if you consent — the tick box “I am happy for a matching partner clinic to contact me.”
If you do not tick that box, we will not pass your name, email, phone or postcode to a clinic.
If you do tick it:
- we may send the fields you submitted to one matching partner (age, condition, format, postcode/ICB geography)
- that clinic may pay this site a fee for a consented introduction — see how we make money
- the clinic becomes a separate controller for what it does next
- this is not an NHS referral and not a guarantee they will accept you
We do not sell lists to data brokers. We do not exclusive-lock you to one clinic in the public directory.
Why we process (lawful bases)

We do not use quiz answers to diagnose you. The quiz is a routing tool against published eligibility tests.
How long we keep it
24 months from last contact unless a longer legal hold applies.
Backups follow the host’s cycle. Email copies sit in the operator inbox until deleted under the same period.
Your rights
You can ask to access, correct, erase, restrict or object to processing, and to data portability, as UK GDPR allows.
Withdraw consent / request erasure: write to PASECOM GROUP LTD at the registered office, or use the get-help form, from the address you used on the form, with the subject “Privacy request”. Say whether you want us to stop clinic sharing, stop updates, delete the form entry, or all of these. If we have already passed your details to a clinic with your consent, you will also need to contact that clinic.
You can complain to the Information Commissioner’s Office: ico.org.uk. We would rather fix the issue first.
Cookies
Separate cookie consent (Complianz or equivalent) covers non-essential cookies. Form pages should still work if you refuse marketing cookies. Essential hosting and security cookies may still run.
Children
The quiz asks whether the person is under 18 because some providers set age rules and because children’s pathways differ in practice. We do not use the quiz to assess a child clinically. A parent or guardian should submit get-help details for someone under 18.
